Peace of Mind Care Management

 Lisa Carle has worked in the health and wellness field for over 30 years. As an Occupational Therapist, Lisa has worked with older adults in skilled nursing and assisted living facilities, retirement communities, as well as in home - health rehabilitation.  

 

Lisa focuses her interventions on her client's values, helping them be engaged with important and meaningful activities and to live with dignity and self - determination.

After witnessing the complexities of aging and disability, Lisa pivoted her career to providing Geriatric and Disability Care Management services. Lisa assists her clients, and their families, to stabilize their care, prevent crises, plan for the future, and navigate the inevitable ups and downs of the aging or disability journey.
